Physical/Health History Update Forms and registration need to be submitted by the following dates:

Physical Due Dates

Fall Sports - Friday, August 21, 2026
Winter Sports - Friday, October 23, 2026
Spring Sports Monday, February 22, 2027

Please submit to the Health Office

Registration Due Dates

Fall Sports - Friday, August 28, 2026
Winter Sports - Friday, October 30, 2026
Spring Sports Monday, March 1, 2027

Please register your child on Powerschool

Prior to participation the following forms need to be submitted to the BRMS Health Office:

Pre-participation Physical Evaluation (PPE) (If your child has not had a sports physical completed
or
Health History Update (If your child has had a sports physical completed by a physician in the last 90 days)

Participation for Travel Sports are Monday through Friday
